Rates & Insurance

PAYMENTS & INSURANCE REIMBURSEMENT:

Clients are expected to pay the standard fees at the time of service. Fees are outlined as follows:

Individual Rates:

$175.00 per initial assessment session.

$150.00 per 45 minute session- individual therapy

$175.00 per 60 minute session- individual extended therapy.

Couples/Relationship Rates:

$175.00 per initial assessment session- private fee.

$175.00 per 60 min session for couples therapy- private fee.

$ 262.00 per 90 min session for extended couples therapy-private fee.

Parent Consultation Rates:

$175.00 per initial assessment session- private fee.

$175.00 per 60 min session for parent consultation- private fee.

$ 262.00 per 90 min session for extended parent consultation-private fee.

At Dragonfly Wellness, couples therapy and parent consultations are always a private fee, and not processed through insurance. Couples therapy focuses on the health and improvement of the relationship which is not medically necessary.

Parental consultations are helpful for challenges with younger children and for parents wanting an opportunity to process helpful behavioral strategies.


Out of Network:  Insurance Coverage

Payments are made at the time of service, and for those who make a request, a Superbill will be provided.  A Superbill is a standard form that we will provide you to submit to your insurance for direct reimbursement to you, after your payment is made in full.  

Services may be covered in full or in part by your health insurance or employee benefit plan, and would fall under out patient mental health benefits with an out of network provider.  

Please check your coverage carefully by asking the following questions:

  • Do I have out of network mental health insurance benefits?
  • What is my deductible and has it been met, and do I have a separate deductible for out of network provders?
  • How many sessions per year does my health insurance cover?
  • What is the coverage amount per therapy session?
  • Is approval required from my primary care physician?

Payment
Cash, check and all major credit cards accepted for payment, and payment is due at the time of service.

Cancellation Policy
If you miss your scheduled therapy appointment, and you have not notified us at least 48 hours in advance, and it is not due to an emergency, you will be charged a $75.00 administrative fee with the credit card that you have on file with our office.
